In the example here, we are entering a patient who has never been entered into the EMR before.  The first time the patient is seen, we enter the patient’s demographics (name, date of birth, etc.) and a first patient record are created simultaneously.  For each subsequent patient visit, the initial demograpic as well as patient medical information (e.g. medication list, list of medical problems, etc.) are brought forward from the most recent visit.
