In this example, we are entering record (a new H&P) on a patient who has been previously seen and entered into the EMR.  Therefore, his demographic information already exists in the system.  For each subsequent patient visit, the initial demographics as well as some of the patient’s most important medical information (e.g. medication list, list of medical problems, allergies, immunizations, etc.) are brought forward from the most recent visit as those items always remain pertinent to each subsequent patient visit.
